NTSB Bridge Failure Recommendations:

Webbers Falls, OK:
A towboat on the McClellan-Kerr Arkansas River veered off course and rammed a pier of the I-40 highway bridge. The impact collapsed a 503-ft section of the bridge, which fell into the river.

Kingman, AZ:
The Southwest Chief train derailed on the Burlington Northern Santa Fe Railway tracks as it crossed a bridge with a supporting structure with no ground support.
9/16/1998 Recommendation Report

Pocomoke City, MD:
An 85-ft section of the Bascule Bridge fell about 30 feet into the Pocomoke River after two pile bents supporting the bridge collapsed.

Covington, TN:
Route 51 bridge over Hatchie River collapsed after two pile-supported column bents supporting three bridge spans collapsed.
7/24/1990 Recommendation Report

Miamitown, OH:
A 140-ft section of the Harrison Road temporary bridge fell into the Great Miami River after a pile bent collapsed.
12/21/1990 Recommendation Report
Mianus River, Greenwich, CT:
A 100-ft long suspended span between piers 20 and 21 of the I-95 highway bridge collapsed and fell 70 feet into the Mianus River.
